Office Hours
Our office staff is available to serve you Monday through Friday 8am-4pm. Please contact us during normal business hours with any inquiries you may have.
Cancellations If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This will make it possible to give your appointment to another patient. Failure to notify the office may result in a service fee of $25.00.
Emergencies Our office provides on call service to our patients for emergencies, 24 hours a day, seven days a week by calling 973/595-7779
Prescription Policy Any request for new prescriptions or refills must be called into our office during our regular business hours. NO prescriptions will be given or refilled after hours or during the weekends. Please allow 24 business hours for your request to be processed.
Insurance Acceptance Our office participates with most insurance plans, including Medicare, Workman's Compensation and No Fault. Please contact our office to see if your insurance plan is accepted.
Financial Policy The following items are required to process your visit :
- Valid Insurance ID card
- Referrals if your plan requires
- COPAYMENTS
- Any deductible or coinsurance that may apply
Self Pay Patient Policy Any patient without insurance will be responsible for payment in full at the time services are rendered. No discounts will be offered unless the entire balance is being paid in full.
